Uso de Profilaxis Preexposición por Mujeres Transgénero: Revisión Integrativa

Resumen

Introducción: Las mujeres transgénero son más vulnerables a las infecciones de transmisión sexual, especialmente al Virus de Inmunodeficiencia Humana. En este contexto, la Profilaxis Preexposición es una estrategia de prevención eficaz y basada en la evidencia.

Objetivo: Analizar la evidencia científica sobre el uso de la Profilaxis Preexposición en mujeres transgénero.

Métodos: Se trata de una revisión integrativa que utiliza un marco metodológico registrado en Figshare y una pregunta guía basada en la estrategia PICo. La búsqueda de estudios se realizó en las siguientes fuentes de información: CINAHL, LILACS, PubMed, SCOPUS y Web of Science, utilizando descriptores controlados y palabras clave. Se utilizó la Clasificación de Niveles de Evidencia y tres instrumentos para evaluar la calidad metodológica.

Resultados: Se incluyeron 47 estudios en esta revisión. De estos, 28 fueron publicados en los Estados Unidos de América, 12 en el Reino Unido, cinco en Suiza, uno en Canadá y uno en Australia, todos en inglés y se publicaron entre 2015 y 2024. Se encontró que 15 de ellos fueron cuantitativos y transversales, tres fueron diseñados como una cohorte y uno fue un ensayo clínico aleatorizado, 23 fueron cualitativos y cinco tuvieron un diseño mixto. Se desarrollaron cuatro categorías temáticas: conocimiento, uso o interés en la Profilaxis Pre-Exposición por mujeres transgénero; facilitadores para el uso de la Profilaxis Pre-Exposición; obstáculos para el uso de la Profilaxis Pre-Exposición; y estrategias para mejorar el uso de la Profilaxis Pre-Exposición.

Conclusión: Es evidente que, si bien la Profilaxis Pre-Exposición representa una estrategia efectiva en la prevención del Virus de Inmunodeficiencia Humana, las mujeres transgénero aún enfrentan barreras significativas para su acceso y adherencia, como el estigma en los servicios de salud, la desinformación y las vulnerabilidades socioeconómicas. Se refuerza la importancia de estrategias específicas y políticas públicas inclusivas que promuevan un enfoque equitativo y culturalmente sensible para abordar esta infección en esta población.
